Letter to Patients Announcing Change in Financial Policy
Many of our providers have requested that we provide them with a letter for patients explaining the change in financial policy at their clinics to include ChiroHealthUSA. Below you will find a PNG or JPEG image of a “Change in Financial Policy Announcement.” You may paste this image directly onto your clinic letterhead and stretch it to fill the entire letter. Simply print the letters, sign them and mail them out to your patients. The disclosure information MUST be included at the bottom of this announcement as required under state and federal statute. Therefore, please do not edit the image in any way.

Why is the Discount Challenge prize amount $15,024? Because that is the average “per-occurrence” fine for Medicare inducements. That’s not $15,024 per patient, that’s not per provider, that’s PER VISIT. Stinks, doesn’t it?
